This section discusses how medication information from different sources is aggregated into a medication overview, and how that overview supports medication reconciliation.
A medication overview can be assembled by aggregating medication information that already exists in a patient's records - typically prescriptions, dispenses, administrations and statements - and summarizing it into medication treatment lines. This aggregated view is a key input to medication reconciliation, where the medications a patient is actually taking are compared with what has been prescribed and dispensed, and discrepancies are identified and resolved.
Different kinds of medication lists arise depending on how and when they are produced - for example an active medication list, a discharge list, a reconciled list, or an aggregated view. A defined set of list-type codes is being developed by the expert group and will be provided, together with the corresponding ValueSet, in a future version.
